Participation in community- Is a way of helping participants with acquiring , retaining , or improving self help, socialization , and adaptive skills necessary to live successfully in the community and to participate in home and community life. Individualized activities are consistent with achieving the outcomes identified in the person-directed plan and are designed to reinforce therapeutic outcomes targeted by other service components.
The activities you may choose from include  joining a club, playing sport , or going to the gym. You can also try other new hobbies such as dancing, surfing, drama, photography, or music classes.
If hanging out with some friends at BBQs or movie nights sounds more appealing then we are here to help make the most out of your leisure time. 

Community Nursing Care

Lifepath Care Services provides home nursing disability care services to prevent illnesses  and promote a healthy lifestyle.

NDIS Nursing Services provides people from many cultural backgrounds often with disadvantaged and marginalised people the best of healthcare. Our registered nurses identify barriers to healthy lifestyle and general wellness as they work with families and communities to empower individuals. They also provide post acute care to people in their homes. Not only do they possess impeccable communication skills, but they also maintain patience in clinical settings and off-site like doing home care.

Accommodation(Supported Independent Living)

Lifepath Care Services is committed to help and /or supervision of daily tasks to help participants live as independently as possible while building their skills. With 
accommodation support our goal is to provide you with daily tasks to assist you in living independently. Our qualified support workers will support you to achieve your daily tasks like cleaning , laundry, cooking ,  balancing your nutrition , showers and all other activities of daily living  as well as  attending your doctor's  appointments and taking your medications timely. All the care provided will factor in your input in your care. 

Group centre activities

Lifepath Care Services  promotes socialisation while offering both workers facilitated and self-directed building activities.  We will help you undertake activities independently and in workshop as well as to encourage contributions  in weekly planning. All activities are participant directed. Our programs facilitate practical learning opportunities for life skill development and support clients to explore personal interests.

Our program activities include;  Improved health and wellbeing such as group fitness activities, walks, swimming, yoga and  mindfulness.  You will be able to participate in dancing, arts and crafts, bowling, dancing and music for recreational purposes.

Assist -personal activities

Lifepath Care Services  understands that it can be quite difficult having someone you don't know supporting you with personal care.

Support may include someone being nearby in case of a fall or an emergency while taking a shower or helping with personal hygiene where needed and taking the  right medications prescribed .

Lifepath Care Services have support workers who are experienced  and passionate about their work. They will assist you with all the tasks , tailored to your specific needs.  

Mental Health Care

At Lifepath Care Services we have a passion for supporting participants living with various disabilities. Which include the following;

Psychosocial disabilities such as depression , bipolar disorder, complex grieving , social phobias , agoraphobia, Schizoaffective disorder Schizophrenia , obsessive compulsive disorder (OCD) ,  post -traumatic stress disorder (PTSD) , anxiety  and major depressive disorder (MDD)

Intellectual disabilities such as down syndrome , autism spectrum disorder (ASD) , developmental delay, fetal alcoholism spectrum disorder (FASD) and ADHD.

Neurological disabilities such as stroke, learning disabilities, ADD , cerebral parsley , epilepsy , neuromuscular disorders dementia , Parkinson’s disease , motor neurone disease , acquired brain injury (ABI) multiple sclerosis (MS) and spinal conditions.

Physical disabilities

Arthritis and musculoskeletal disorders and  spinal cord injuries . 

Assist -travel / transport

Lifepath Care Services provides travel and transport services to people with disability. Assistance with travel and transport include supports that enable participants to build capacity to independently travel, including through personal transport-related aids and equipment, or training to use public transport.

A participant’s transport supports may also include the reasonable and necessary costs of taxis or other private transport options for participants who are not able to travel independently, as well as transport to and from school for students.

Lifepath Care Services staff will drive you to your appointments or another place in the community including work, local shopping centre and take you back home when you’re done. Our transport services are scheduled on an ongoing basis, such as a weekly trip to the shops or regular medical appointments.

All our staff have a valid Working with Children Check and Police check and valid qualifications to work in this Industry.

Daily tasks/ Shared living

Lifepath Care Services understands that starting out to live alone or sharing accommodations can sometimes be challenging. We are committed to helping you live independently or share accommodation that is most suitable for you.  Whether you are living in your own home, living a shared accommodation arrangement or a rental property we can ensure that we’ll assist in providing you with a plan for you to achieve living independently. Our highly trained staff will support you by; teaching you how to live independently and assist you with basic tasks like preparing well balanced meals, assisting you with activities of daily living and making life more meaningful. We will also work with you in transitioning to live with others and ensure you can cope with your responsibilities around the home. Last but not least, we will assist with your move if it is the first time you are going to be living on your own or shared accommodation.

Innovative community participation

Lifepath Care Services will enhance your participation  in the community thus  improving your well-being by  feeling  connected to people and the community. Improved well-being also affects different aspects of life including education, employment, health, satisfaction, and your well-being . We will help you learn new skills or build on your existing skills to give you more independence in your life. Lifepath Care will help you socialise and make connections and  create a network of friends  and find out about new opportunities  that you can be involved in to increase your independence and ability to live on your own and look after yourself. 

Development life skills

Lifepath  Care Services will help you live as autonomously as possible through the growth of skills such as communication ,  which allows you to interact with others and verbalise your needs easier ,  perform daily activities and household chores independently , perform   personal hygiene and  critical thinking thus promoting your independence and  confidence . 

Household tasks

With Lifepath Care Services we will work with you and formulate a plan that enables you to maintain clean surrounding daily such as preparing meals , vacuuming , feeding pets , doing laundry or weekly such as taking out trash , washing bedding , mopping the floor or monthly such as changing air filters on furnace or air conditioner , bathing pets , clean blinds and with yearly chores such as shampooing the carpets, winterize the house among others.
